Adaptive Equipment Application
The purpose of this fund is to make access to appropriate adaptive equipment possible for children with disabilities and special needs. The primary goal of the equipment should be to help enhance the applicant’s opportunities for recreation, mobility, and everyday living success. The review board’s mission is to provide awards to as many individuals as possible. Because funds are limited, not all applicants will receive an award.
Who May Apply
Children and young adults under the age of 26 who have a disability or long-term injury that impacts their everyday living may apply. Each request is reviewed on an individual basis. Applications may be granted with full or partial funding depending on financial need. Many factors are considered during the decision-making process, including the availability of funds at the time the request is received.
If your application is approved and funding is granted, you must wait two years before submitting another application.
How to Apply
To apply for adaptive equipment assistance, please complete the following steps:
Complete the Application Form.
Obtain a Letter of Support from a physician, therapist (such as a speech or occupational therapist), therapeutic recreation specialist, psychologist, assistive technology professional, or teacher indicating that the requested equipment aligns with the applicant’s goals and abilities.
Include a copy of the quote or invoice for the equipment or technology being requested.
Submit the completed application form, equipment quote, and letter of support by email or mail.

Distribution of Awards
Applicants will be notified by letter, email, or phone by a member of the Chase’s Warrior Foundation board regarding the decision to either grant or deny funding for the requested equipment. Awards are distributed until available funds are exhausted. The CWF Board meets once each month, and applications are reviewed during the meeting following the month in which they were received.
Please note that CWF does not provide cash awards. All approved payments are sent directly to the equipment vendor.
