The primary purpose of this equipment should be to enhance the applicant\u2019s potential for recreation, mobility, and everyday living success. The mission of the review board is to provide awards to as many individuals as possible. Funds are limited and not all applicants will receive an award.[\/vc_column_text][\/vc_column_inner][\/vc_row_inner][vc_row_inner][vc_column_inner width=&#8221;1\/2&#8243;][vc_column_text css=&#8221;.vc_custom_1610004151477{margin-bottom: 0px !important;}&#8221;]<\/p>\n<h3 style=\"color: #1c4094;\">WHO MAY APPLY:<\/h3>\n<ul class=\"pagelist\">\n<li>Children and young adults under the age of 26 who have a disability or long-term injury that challenges their everyday living.<\/li>\n<li>Each request is reviewed on an individual basis. Requests may be granted with full payments or partial payment dependent on financial need. Many factors are included in the decision-making process and it also depends on what funds we have available at the time of the request.<\/li>\n<li>If your application is granted and you receive funds, you will have to wait 2 years to re-apply.<\/li>\n<\/ul>\n<p>[\/vc_column_text][\/vc_column_inner][vc_column_inner width=&#8221;1\/2&#8243;][vc_column_text css=&#8221;.vc_custom_1607064874672{margin-bottom: 0px !important;}&#8221;]<\/p>\n<h3 style=\"color: #1c4094;\">TO APPLY:<\/h3>\n<p>1. Complete the attached Application Form.<br \/>\n2. Obtain a Letter of Support from a physician, therapist (e.g. a speech or occupational<br \/>\ntherapist), therapeutic recreation specialist, psychologist, assistive technology<br \/>\nprofessional or teacher that indicates the equipment requested is consistent with the<br \/>\ngoals and abilities of the patient. A Letter of Support Form is attached.<br \/>\n3. Include a copy of your quote\/invoice for the equipment or technology requested.<br \/>\n4. Awards are granted until the funds are exhausted. The CWF Board meets once a month.
